Add an Incoming Webhook to your Teams channel
In Microsoft Teams, open the channel → **... → Workflows** (or **Connectors** in classic Teams) → **Incoming Webhook**. Name it `Open Humana` and copy the generated URL.

Wire it up via Zapier or Make
Create a Zap (or Make scenario) with **Open Humana → Call Transferred** as the trigger and **Webhooks → POST** as the action, pointing at the Teams URL.

Format the message card
Use the sample Teams Adaptive Card payload from the Open Humana docs to format a clean notification.

Activate the Zap or Make scenario. The next transferred call will land in your Teams channel within seconds.
